High-tech won’t save healthcare—Jefferson students will! Students in interdisciplinary teams at Jefferson University are using “hotspotting” to improve patient outcomes and bring down the cost of healthcare. Hotspotting employs innovative ways to help patients with high-care needs caused by multiple, complex chronic conditions, often coupled with severe functional limitations.
Traditional healthcare solutions to address acute or chronic illnesses or injuries have proven inadequate in meeting needs of patients. Hotspotting is a coordinated care plan tailored to the unique needs of each high-needs patient that combines primary care, specialist and chronic disease care, and social services.
